Granuloma and pseudogranuloma of the skin due to Microsporum canis. Successful management with local injections of miconazole

A case in a 25-yr-old immunosuppressed woman with systemic lupus erythematosus is reported. M. canis was isolated from a skin lesion on her left forearm. Treatment with griseofulvin and ketoconazole was unsuccessful. Intranodular injection of 20 mg miconazole nitrate to one nodule at a time every other day was well tolerated. After 1 month the nodules had regressed in size and cultures of aspirated material were negative. The patient was maintained on a regimen of daily griseofulvin and although the forearm remained quite scarred, no further exacerbations occurred during the 3-yr follow up period.
