Insulin resistance in chronic liver disease: the possible role of anti-insulin factors

Hyperinsulinism, insulin resistance and high levels of glucagon, GH and FFA are being recognized in chronic liver diseases. In order to determine which of these endocrinological-metabolic changes were related to liver cell damage, the Authors investigated any possible correlation between hepatic function (BSP retention percentage at 45 min) on the one hand, and insulin sensitivity (evaluated by using the coefficient of glucose disappearance rate -Kitt-) and basal circulating levels of IRI,C-peptide, glucagon, GH and FFA on the other. Forty patients (8 affected by chronic persistent hepatitis, 9 by chronic active hepatitis and 23 by liver cirrhosis) were examined. Our results appear to indicate that altered hepatic function may be responsible for the increase of some anti-insulin factors (glucagon, GH, FFA).
