Rupture of arterial anastomosis after kidney transplantation

In a series of 1,000 kidney transplantations, 14 cases (1.4%) of acute rupture of arterial anastomosis were observed. The associated conditions included: local infection in 8 cases (57%), urinary fistula (4 cases, 28.5%) and infected hematoma (4 cases, 28.5%). Other possible causes were: non-infected hematoma (3 cases, 21%), lymphocele (1 case, 7%) and undetected (2 cases, 14%). Transplant nephrectomy was performed in 2 cases (14%) with good results and conservative measures in 6 cases (43%) with poor results. Five patients died before any treatment could be done. One case of external iliac artery ligature had to be submitted to femoro-femoral bypass.
