Extracorporeal lung assist (ECLA) by intermittent veno-venous bypass for newborn respiratory failure

For a safer and simpler respiratory support with an artificial membrane lung for the newborn, a new venovenous extracorporeal bypass circuit was designed. A thin walled venous catheter was inserted through the jugular vein into the right atrium of puppies and served as a to-and-fro blood access. A rotary pump was activated intermittently when the reservoir was refilled. For the regulation of the blood flow, capacitance detectors were applied to the reservoir. Extracorporeal lung assist with the above circuit was tried with deliberate hypoventilation or lung damage induced by hyperhumidification. The maximal flow rate reached 274 ml/min and PaCO2 [arterial blood CO2 tension] was maintained within the normal range. Venovenous extracorporeal lung assist with a single venous catheter can well become a means of excellent respiratory support for the newborn in the future.
