Plasma functional antithrombin III (AT III) activity was measured in 215 stroke patients and 100 healthy control subjects. The age range of the patients and the controls was between 40 and 79 yr. The stroke patients were divided into 3 groups by CT findings: Group I cerebral hemorrhage, group II deep small infarct, and group III cortical-subcortical infarct. Blood samples were taken within 7 days after the onset of stroke and before any anticoagulant was given. The results were analyzed separately at 10-year intervals between 40 and 79. No difference in AT III activity between 2 sexes was found. The data from analysis of variance in each age interval showed that group I, group II and control group yielded no statistical difference in AT III activity, whereas group III compared with group I, group II and control group showed significantly lower mean value. Patients with cortical-subcortical infarct are associated with low AT III activity at acute stage of stroke. It needs further investigation to find out the actual causes of this phenomenon.
