In 60 patients with metabolically well balanced diabetes mellitus the localization and intracellular activity of beta-glucuronidase have been estimated using the semiquantitative cytochemical method of Hayashi et al. in peripheral blood lymphocytes. In comparison to the control group consisting of 67 healthy subjects of similar age and sex distribution the patients showed several hundred times less lymphocytes having intact enzyme-positive lysosomes. Lymphocytes having no enzyme-positive lysosomes and only cytoplasmatic localization of beta-glucuronidase have been 2 times higher in the patients than in the controls. Alterations of activity and intracellular localization of beta-glucuronidase in lymphocytes occurred both in the patients with a without infectious complications and the alterations are in close relationship to metabolic disturbances in diabetes mellitus.
