Analysis of blood flow pattern in the right heart after Fontan procedure

The velocity of blood flow in the right heart was obtained by a catheter-mounted electromagnetic velocity sensor in 5 cases after Fontan procedure and the pattern was analyzed. Right atrial and left ventricular volumes were also obtained by biplane cineangiography in each case. In the main pulmonary artery, a relatively sharp forward flow (A wave) was detected immediately after the onset of P wave of ECG, followed by a slow forward flow which persisted throughout atrial relaxation. There was no significant backward flow detected at this site. In the inferior vena cava a sharp backward flow was detected at the same as the A wave, followed by a sharp forward flow which began in the QRS complex of ECG and the duration of which was short. Then a slow forward flow followed it. The flow pattern was similar in the superior vena cava. Right atrial volume ranged 77-330% of normal with the mean of 153 .+-. 101 (SD)%, and the systolic volume change was 0.24 .+-. 0.06. These parameters did not correlate to the flow pattern. Left ventricular end-diastolic volume was 110 .+-. 25% of normal and the ejection fraction was 0.53 .+-. 0.08.
