Microcirculatory changes in the lungs in transfusion microembolism

The method of contact microscopy under LYUMAM-K-1 microscope was used to investigate the microcirculation and structure of the lungs after i.v. administration of thromboleukocytic aggregates from autologous and homogolous blood preserved in the TSOLIPK [Central Scientific Research Institute of Hematology and Blood Transfusion] 7B solution for 10-14 days. Vascular embolism is attended by mosaic reorganization in microcirculation manifested in varying periods. Initially, the areas with sufficient ventilation, but deprived of circulation, prevail; 30 min later, individual alveoles demonstrate areas with intensified microcirculation, and there appear pleural microvessels that are not apparent at the initial stage. In venules and arterioles, the circulation slows down to the extent of stasis. After the 1st h the above described phenomena are less manifest, yet there evolves perivascular, interstitial and alveolar pulmonary edema. The causes of the microcirculatory changes are discussed.
