Nature of the proteinuria in multiple myeloma

The analysis of the urine was made in 211 multiple myeloma patients (G in 122, A in 55, and Bence-Jones in 34). G- and A-myelomas were characterized by proteinuria. Three variants of paraproteinuria, independent of the type of the L chains and the content of paraprotein in the blood, were distinguished: MLC (monoclonal light chains), MLC + IgG (or IgA) and IgG (or IgA). It has been shown that separate MLC is infrequently isolated (even in cases with Bence-Jones myeloma). As a rule both low-MW and high-MW proteins are detected in the patients' urine. Most patients with any form of multiple myeloma may have highly permeable glomerular filters that promotes the excretion with urine of high-MW proteins (immunoglobulins, transferrin).
