Percutaneous lung biopsy in mitral valve disease

Percutaneous lung biopsy was done in 65 cases of mitral valve disease. In 51 cases where adequate lung tissue was obtained, histopathological features were studied and an attempt was made to correlate these features with clinical grading of pulmonary hypertension. Most patients were young and had duration of symptoms less than 3 yr. All patients showed varying degrees of vascular and parenchymal changes. In the musculature, branches of pulmonary artery media was thickened to varying degrees. Fibrinoid necrosis was seen in 4 cases and evidence of angitis in 2 cases. Intimal proliferation, reduplication and various stages of thrombosis were also seen. Other changes included hypertrophy of veins, dilatation of lymphatics and variable degree of alveolar wall thickening. There was fair correlation between histopathological grading and other parameters. In few cases when the left atrium is not enlarged the pulmonary vascular changes occur earlier and are severe.
