Hemodynamic effects and clinical observations on prazosin in treating chronic refractory congestive heart failure

Patients (17) with chronic congestive heart failure (CHF) were studied by using Swan-Ganz catheter after taking the oral vasodialtor prazosin. The blood pressure, the mean right atrial pressure, the pulmonary arterial pressure decreased and the cardiac output increased after a single dose of 3 mg. These effects appeared at 1 h after taking this drug and sustained for 6 h. Patients (47) with CHF were treated with 3-16 mg proazosin/day. The symptoms ameliorated and the cardiac functional class improved. Of 47 patients 26 had good response in 1 mo. Thirteen of 17 patients and 4 of 5 patients had satisfactory response within 1/2 a year and in 1 yr, respectively. Application of prazosin hydrochloride is convenient for outpatients with CHF for short and long term therapy.
