A successful Jatene operation in a 8-month-old infant with Taussig-Bing anomaly

An 8-mo. old infant with Taussig-Bing anomaly associated with preductal coarctation underwent 2 stage anatomical correction with success. After coarctectomy at 6 mo. of age, complete correction was performed with arterial switch procedure (Jatene operation). The aorta was anterior to the pulmonary artery and a ventricular septal defect was subpulmonic with 60% overriding. After the dissection of both coronary ostia from the aorta with large U-shaped pieces of aortic wall, coronary arteries were transposed to the posterior artery. The ventricular septal defect was closed through right ventriculotomy. The pulmonary artery was reconstructed interposing a 12 mm diameter Gore-Tex graft. Postoperative course was satisfactory and the patient is doing well 10 mo. after surgery. Indications for and the method of Jatene operation were discussed.
