The changes in serum and urine K before, during and after a use of cold K cardioplegic solution was investigated in patients who underwent open heart surgery. Patients (19) were divided into 3 groups: 5 patients with ASD bypass operation with a use of cardioplegic solution, collected into the circuit of cardio-pulmonary bypass. The serum K level apparently does not elevate inordinately even in the group of A-C bypass patients. Their peak level (4.35 U 0.64 meq/l) was present during assisted perfusion after multidose infusions of cardioplegic solution. The urinary excretion rate of K increased during and after infusions of cardioplegic solution. Especially, the increase of urinary excretion of K in the group of A-C bypass patients was significantly greater than in the control group of ASD patients. The amount of K administered as a cardioplegic solution in the group of A-C bypass patients was excreted into urine within 6 h after the cessation of cardiopulmonary bypass. The K cardioplegia is a safe method of myocardial protection and shows no deleterious systemic hyperkalemic effects.
