Iatrogenic, accidental, complete transsection of the brachiocephalic trunk and immediate successful reconstruction by interposing the great saphenous vein

In an 22 years old woman a complete transsection of the truncus brachiocephalicus occurred during tracheotomia. The bleeding was interrupted by Kocher-clamps, which led to a traumatisation of the ends of the cut vessel. Therefore, an end-to-end anastomosis could not be performed and saphenous vein of 3 cm length was used to bridge the defect. Follow-up 5 years later: no complaints, no difference of blood pressure, no stenosis. Complete recovery. A control angiogram could not be performed since the patient gave no consent.
