Interest for colonic cancer prevention of the follow-up of patients after endoscopic resection of colorectal polyps: 123 cases

Following endoscopic resection of a colorectal polyp, patients belong to a high risk population and require increased surveillance. The risk of progression of the disease in these patients was evaluated. From a population of 309 patients with rectal and/or colonic polyps diagnosed between 1963 and 1978, 263 subjects with 358 adenomatous polyps treated by complete endoscopic resection were studied. Of these, 123 (47%) were examined a 2nd time, during which a complete colonic scan was conducted. As a result, 81 new polyps were detected in 47 patients, giving a recurrence rate of 38%. A statistical study of different parameters (age, sex, number of polyps, histological type of the polyp) showed no significant difference between the populations with or without recurrence. Comparison between the histological type of the initial polyp and that of the recurrent confirmed a remarkable stability of the lesions. Recurrences in 18 of the 19 cases with polyps showing mild dysplasia were of the same type. Similarly, in the group with polyps showing marked dysplasia or malignant changes, recurrences were of the same type in 17 cases of 20. Finally, comparison of the number of recurrences after 1 yr, 3 yr and 5 yr showed a marked increase which must be taken into account when establishing surveillance procedures. This study confirms the necessity for follow-up of this population, but does not allow identification of significant risk factors.
