Colonic polyposis associating juvenile and adenomatous polyps, with adenocarcinomas developed within both types of polyps

A 29-yr-old Tunisian female, without relevant past medical history, was operated on as an emergency for an obstructive syndrome. A cecal tumor, with peritoneal metastases and multiple polyps, was discovered, and a right hemicolectomy was performed. The pathological specimens showed an infiltrating adenocarcinoma arising from a tubulovillous adenoma of the cecum, several juvenile and adenomatous polyps and polyps with both juvenile and adenomatous features. A superficial carcinoma was discovered in the pedicle of a typical juvenile polyp. A few cases of adenocarcinoma associated with juvenile and colonic adenomatous polyps have been reported in the literature, but in no case had the adenocarcinoma developed from a juvenile polyp. These findings suggest the necessity, apparently in juvenile polyposis, of endoscopic excision of as many polyps as possible for anatomopathological study, to detect either associated adenomatous polyps or a possible carcinoma arising in a juvenile polyp.
