Total parenteral nutrition (TPN) in the management of post-operative alimentary tract fistulas

Report is of 30 persons with a fistula following gut surgery who had total parenteral nutrition (TPN) for 8 to 120 days. The fistula closed for 27 persons; it closed for another 1 person after further surgery, and 2 persons died owing to the persistence of the fistula. Direct surgery of the fistula does not assure its closure, because of hypercatabolism and sepsis. Both those factors can be modified using TPN. Failure of TPN is conditioned by the site and rate of leaking from the fistula.
