The electrophysiological effects of orally administered diltiazem hydrochloride were studied. Diltiazem was administered (180 mg/day) to 10 subjects with normal ECG who had never manifested signs of sinusal dysfunction or nodal conduction and to 10 patients (120 mg/day) who manifested the basal state of anomalies in sinusoidal function. Diltiazem had no depressive effect on sinusoidal function or atrioventricular function in the 1st group. In the 2nd group, diltiazem lengthened the time of sinusoidal recuperation for nodal conduction and the nodal refractory periods. Severe anomalies of sinusoidal function contraindicate the use of the drug, and intranodal atrioventricular conduction disturbances should indicate caution. Intraventricular conduction disturbances are not aggravated by diltiazem.
