First cases of botulinic paralysis in Puerto Rico

In August, 1978, three cases of botulism (one fatal) associated with ingestion of marinated fish occured in Puerto Rico. This is the first known outbreak of botulism on the island. The fish prepara tion, known as "sierra en escabeche" is locally quite popular, and its high vinegar content should make the growth of Clostridium botulinum unlikely. Type A botulinal toxin was identified in the suspected f000d product and in the serum of two of the three patients; pH of the fluid phase of the food was under 4.6.
