Vaso dilatory response of coronary artery to nitroglycerin in patients with and without coronary spasm

There has been increasing interest in the phenomenon of reappearance of coronary artery spasm intra or after bypass surgery in the patients of vasospastic myocardial ischemia. Sixteen surgical cases with provoked coronary spasm superimposed on atherosclerotic lesions were studied. These cases include 8 patients with variant angina, 7 patients with unstable angina and 1 patient of old myocardial infarction. Sixty-six nonsurgical patients served as controls. Cases (23) of variant angina, 11 cases showing angiographic coronary spasm without diagnosis of variant angina and 32 cases without vasospastic disease were investigated. On all these cases, coronary diameter in each segment were measured angiographically before and after administration of nitroglycerin, and the increasing ratio of the coronary arterial caliber were calculated as the percent change of coronary dilatation (.degree. .DELTA. CD). In the cases of nonsurgical candidates, .degree.DELTA. CD was 0.33 .+-. 0.11 in vasospastic cases, and 0.15 .+-. 0.06 in non-vasospastic cases. There was no difference in .degree.DELTA. CD between the cases of variant angina and those of nonvariant form but angiographically coronary spasm. No significant correlation was demonstrated between .degree.DELTA. CD and number of lesions of coronary atherosclerosis in either the group of spastic or the group of nonspastic coronary arteriogram.
