Our understanding of paroxysmal nocturnal haemoglobinuria (PNH) has improved considerably in recent times. This stem cell clonal disorder develops presumably through somatic mutation. It is certain that a high degree of suspicion in cases of refractory anaemia, unexplained chronic haemolytic anaemia, recurrent venous thrombosis and aplastic anaemia will be necessary to detect cases of PNH in our country, where various nutritional deficiencies may mask many of the classical features.
