Anomalous origin of left coronary artery from pulmonary trunk with mitral insufficiency

A 5-yr-old girl with anomalous origin of left coronary artery from pulmonary trunk associated with mitral insufficiency underwent saphenous vein graft surgery successfully. Autologous saphenous vein graft was interposed between the ascending aorta and the stump of the anomalous left coronary artery and mitral insufficiency was left untreated expecting decrease in its severity after improvement of left ventricular function. Moderate decrease in mitral insufficiency and patency of the saphenous vein graft were observed in the postoperative catheterization study. The wall of the anomalous left coronary artery was irregular and partially forming an aneurysm. After catheterization the patient was suddently attacked by acute angina, probably due to coronary thromboembolisms in the anomalous left coronary artery. In retrospect, anticoagulant therapy should have been performed immediately after the operation. The patency of the saphenous vein graft and the severity of the mitral insufficiency should be carefully observed on a long-term basis.
