Mallory-Weiss syndrome. Considerations on 27 cases

Twenty-seven patients, inspected by endoscopy and diagnosed as having the Mallory-Weiss syndrome, were studied, taking into account their age, sex, background, clinical presentation, manifestations, number of lacerations, associated lesions and evolution. Twenty-three of them were males and 4 females. The average age was 46.7 yr. Only 8 patients had increased intraabdominal pressure, 7 of them suffered from retching and vomiting, while one had an excessive cough. Out of the 21 patients that were treated, 9 were chronic alcoholics while 3 were previously ethyl-intoxicated. Prior ingestion of salicylates had taken place in 6 patients. The clinical presentation of 22 of them was gastrointestinal bleeding, i.e., 4.9% of all the upper endoscopies carried out. Single lacerations were present in 22 cases, double ones in 4 and a triple in 1. Frequently found were associated endoscopy lesions, the most common one (37%) was a hiatal hernia. They all were medically treated except one patient, who was operated on because of an associated gastric perforation. Just one of the Mallory-Weiss syndrome patients died, due to an associated diffuse bleeding gastritis.
