A case is reported in an 18-yr-old Mexican man, now living in USA. M. mycetomatis was cultured from black granules in a biopsy specimen taken from multiple draining sinuses at the sites of insertion of Ender nails for a closed tibial fracture. No therapy was given and it is suggested that amputation may be necessary if the infection progresses.
