Effect of acetazolamide on acute respiratory alkalosis

Acetazolamide is useful for the treatment of respiratory alkalosis caused by prolonged mechanical ventilation. Some cases with severe respiratory alkalosis which did not react to various medications occasionally could be recovered with 5 mg/kg per day of i.v. acetazolamide. Experiments determined the clinical effects of acetazolamide. Adult mongrel dogs were anesthetized and ventilated with volume limited ventilator and the following indices were analyzed: blood pressure, EEG, urinary volume, blood and CSF gases and electrolytes (Na, K, Cl) in arterial blood, CSF and urine. By the administration of acetazolamide in dogs with respiratory alkalosis: arterial and CSF [H+] were corrected; blood and CSF electrolytes were restored and maintained within normal limits; arterial blood pressure, pulse rate and EEG were kept stable; and urinary volume increased. I.v. 5 mg/kg acetazolamide was safe and useful for the treatment of respiratory alkalosis.
