Platelet support in patients undergoing bone marrow transplantation

Eighteen patients undergoing bone marrow transplantation (BMT) were given 231 platelet transfusions over 785 patient/days. Mean number of transfusions/patient was 12.8 (range 4-29) with a frequency of 1 transfusion every 3.4 days (range 1-7.3). Mean increments/cmm per xm2 per platelet unit were 6164 .+-. 6188. Men increments decreased in patients receiving over 10 transfusions, though SD were quite high and differences not statistically significant (P > 0.1). Similarly there was no significant correlation between increments and time of platelet support (P > 0.1). High frequency of transfusions correlated significantly with low platelet increments (P < 0.001). Of 231 transfusions, 128 were obtained from HLA-typed donors: retrospective analysis of these transfusions showed that HLA-matched platelets had little effect in patients receiving multiple random transfusions as well. Refractoriness to platelet transfusions developed in 14% of patients receiving less than 10 transfusions and in 80% of patients receiving over 10 transfusions. These results and the refractoriness to HLA-identical allogeneic and syngeneic platelets are discussed.
