High positive end expiratory pressure and low rate of intermittent mandatory ventilation for hypoxemia

High positive end-expiratory pressure (PEEP) with low rate intermittent mandatory ventilation (IMV) was introduced for treatment of severe hypoxemia. Four patients with extreme hypoxemia (PaO2 .apprx. 20 torr) treated by this method are presented. Low rate IMV is important in reducing the incidence and severity of the complications resulting from high levels of PEEP. The problem of hemodynamic monitoring and the hazards of administration of colloid solutions to these patients are discussed. Full recovery of pulmonary function was observed in 3 patients in whom pulmonary function was tested.
