A multiple approach to the treatment of obesity using total fasting or a very-low-calorie diet

A multidimensional therapeutic program was used in the treatment of over 20,000 obese patients (15% of whom had been hospitalized) over 19 years. The program aims to help the obese patient change his life style. Staff included an interdisciplinary clinical team of specialists in psychology, biochemistry, dietetics, radiology, cardiology, occupational and physical therapy, statistics, and medicine. A diagnostic-prognostic profile is defined for each patient. Ambulatory treatment (a food plan, psychosomatic interview, and physical activity through individual or group approach) and/or hospitalization with total fasting or a very-low-calorie diet are prescribed, enabling patients to achieve moderate weight loss which can be maintained. The overall program also includes group therapy, physical training, use of video tapes, occupational therapy, and family guidance. The clinical center does follow-up on patients by mail and phone; clinic activities and peripheral groups provide continuous support and motivation. (nm)
