Temporomandibular joint pain in a renal dialysis patient

A 21 yr old woman on renal dialysis and sodium warfarin therapy developed swelling over the right temporomandibular joint, displacement of the right condylar head and an open bite. The condition was diagnosed as one of joint effusion, probably of hemorrhagic origin. I.m. injection of vitamin K and the cessation of sodium warfarin therapy was followed by resolution of the symptoms over 4 days. This patient emphasizes the need for taking a careful and complete history and the evaluation of all diagnostic aids so that a reasoned diagnosis can be made.
