Surgical treatment of sympathetic cervical spondylosis by the anterior approach is recommended in patients not responding to conservative treatment. Of 15 (5%) operated cases, the results were good in 7, fair in 7 and poor in 1. Mechanism, diagnosis and treatment are discussed.
