Physiopathological aspects of the hypotensive effect of surgery in renovascular hypertension

In 21 patients with vasorenal hypertension caused by unilateral stenosis of the renal artery, changes in the central hemodynamics and blood renin activity, which depend on the degree of hypotensive effect produced by the operation, were studied. No changes in the characteristics tested were found in patients in whom the surgical treatment proved ineffective. The main hemodynamic factor that determines the decrease in arterial pressure after the operation is the diminution of the peripheral vascular resistance that occurs concurrently with the fall of blood plasma renin. If the operation resulted in a moderate hypotensive effect, an additional hemodynamic factor of the regression of the hypertension syndrome is the decrease of the magnitude of cardiac output and circulating blood volume.
