Isthmic aortic coarctation: the effect of corrective surgery on systemic arterial pressure

Systemic arterial pressure values were studied in 25 patients, ages ranging from 7-54 (average 26), who were operated on for resection in the isolated isthmic coarctation of the aorta. A significant reduction in systolic and diastolic values resulting from the operation were confirmed in all patients; 48% of the subjects had normal pressure when discharged. At the clinical check-up 1-7 yr after the operation, the percentage of patients with normal blood pressure was lowered to 12% with a positive relation between pre-operative pressure values and later ones. Blood pressure on discharge from the hospital after the operation was not a reliable indication of pressure behavior in the follow up. In the 8 patients who underwent the treadmill test (compared to normal subjects) there was a moderate but significant increase in systolic and diastolic values. Blood pressure in patients examined and in various conditions did not depend on the age of the patient at the time of the operation. Surgical treatment for arterial hypertension evidently is higher in subjects operated on between the ages of 4-6. Various pathogenic factors on the basis of residual arterial hypertension were briefly examined.
