Timolol ophthalmic solution for the treatment of glaucoma

Effects of timolol maleate ophthalmic solution (T) were studied in 21 patients (35 eyes) with various types of glaucoma including mainly primary open angle glaucoma. Of 35 glaucoma eyes, 19 demonstrated satisfactory IOP control with 0.25% T alone; 8 eyes showed good IOP control with concomitant use of pilocarpine or carbonic anhydrase inhibitors. Patients in the treatment of T alone produced a statistically significant decrease in the mean IOP after 4 wk of treatment. Of chronic narrow angle glaucoma patients, IOP was controlled only when concomitantly used with pilocarpine. No noticeable adverse reaction due to T occurred during the study. Mild, diffuse superficial keratitis was noted in 1 patient. Pupillary diameter was not affected by any concentration of T. No significant change in pulse rate and blood pressure was shown. A significant decrease was observed on wk 2 in pulse rate (P < 0.01). A drop of T twice a day may be an effective and safe ocular hypotensive agent for the treatment of galucoma. Glaucoma treatment demands a prolonged medication; close observation will be needed for the use of T until a long-term study is done.
