An infant was seen at a hospital in South Carolina, USA, with a history of diarrhoea and vomiting of at least one week's duration. Respiratory difficulty and seizure activity were noted and he died 3 days after admission. Autopsy revealed changes in the brain and meninges. Microscopically, there were patchy areas of necrotizing inflammation, frequently with a zone of prominent granulomatous reaction and collections of large trophozoites characteristic of Acanthamoeba. Wrinkled, double-walled cysts were also seen occasionally.
