Infarct size of the left ventricle. Relation between angiographic and enzymatic assessment

Patients (30, average age 50.8 yr) with typical enzyme and ECG changes during the acute phase of myocardial infarction mass (IS = 25.9% of total LV mass). There was a significant correlation between the IS-ANG and IS determined from CK washout (r = 0.74, P < 0.001). This correlation was improved by using an angiographically determined infarct index (length of akinetic segment .times. wall thickness) instead of LV circumference (r = 0.84 for anterior and posterior wall infarction, and r = 0.88 for anterior wall infarction alone) (P < 0.001). There was a negative correlation between ejection fraction (EF) and IS (r = -0.82, P < 0.001); extrapolated normal EF, 66%; for IS of 10% EF was 59%; and for IS of 40%, 38%. The close correlation between IS-CK and IS-ANG suggests that CK analysis for many cases is a reliable technique, provided correct mathematical approximation is used; in the majority of patients IS apparently does not change during the first 4 wk after acute MI.
