Isolated sarcoidosis of the upper mediastinal lymph nodes

Two cases of atypical localization of mediastinal sarcoidosis are described. The chest X-rays of both patients showed widening of the upper anterior mediastinum above the azygos vein without enlargement of the peribronchial lymph nodes. As Hodgkin's disease and/or thymoma could not be ruled out and mediastinoscopy was considered to be non-diagnostic on account of the localization of the tumor, thoracotomy was performed. Enlarged paratracheal nodes were found. Histological examination revealed sarcoid adenopathy. The isolated involvement of paratracheal lymph nodes without enlargement of the peribronchial nodes is considered a relatively rare condition.
