Correction of protein metabolism disorders in patients with severe burns

Treatment was given to 267 patients, 16 to 67 years old, with severe burns. During the acute stage 132 patients were given transfusions to restore blood plasma values and Tracilol, 150 000 to 300 000 IU daily, or one of its analogues, Contrikal or Gordox, for 7 to 10 days after being burned to inhibit proteolytic enzymes. In most patients bodyweight was maintained or decreased slightly, proteins and albumins in plasma stabilized, protein breakdown products, especially nitrogen metabolites in urine, decreased and wounds healed faster. Used at a later stage, Tracilol and its analogues reduced proteolytic activity, often greatly increased, in patients with an unfavourable disease course including rejection of skin grafts. In 113 patients given fatty emulsions, hypertonic glucose solutions, alcohol and amino acids parenterally during the acute and infective stages to provide about 5000 kcal daily for 8 to 10 days there was an inhibition of catabolic and an increase in anabolic processes. Activity of pseudocholinesterases increased, indicating increased protein metabolism in the liver. Progressive hypoproteinaemia was stopped. In most, skin grafts were well accepted. There were similar improvements in protein metabolism in patients with diabetic-type sugar tolerance given 16 to 32 IU insulin daily for the first 12 days or longer.
