Sex reversal in a case of male pseudohermaphroditism (Reifenstein syndrome)

A case of male sex reversion in a patient presenting ambiquous genitals at birth, hypospadic phallus and gonads in the inguinal area is reported. At age 7, yr he was submitted to orthophaloplasty and neourethroplasty. During puberty, he developed breast and lack of beard. Since childhood he had identified himself with the female sex despite the surgical procedures. After a detailed study for sex identification, a diagnosis of male pseudohermaphroditism (Reifenstein syndrome) was established. The psychological profile revealed the identification of the patient with the female sex and because of his request for an operation to adapt the genitals to this situation, the request was considered justifiable and feasible. The postoperative organic and psychic results were satisfactory.
