Of 38 hemorrhagic diaphragmatic hernias, 2/3 presented as overt bleeding and 1/3 as anemia. Fibroscopy was important for locating the bleeding lesion, which was superficial in 75% of the cases and lay in the neck of the hernia as often as in the lower esophagus. The pathogenic mechanisms were varied and included gastro-esophageal reflux and purely mechanical phenomena. Patients (35) were operated upon, including 3 emergencies. The surgical procedure adopted was conservative and aimed at reducing the hernia and preventing the reflux; it had remarkable and lasting hemostatic effects.
