Patients (58, 116 eyes) with Vogt-Koyanagi-Harada Syndrome were reviewed. This syndrome occurred more commonly in the 3rd and the 5th decades of life. In 92.3% of the patients, the 2nd eye was involved within 3 days after the onset of the 1st eye. In 22 patients diagnosed within 2 wk after the onset and treated with systemic corticosteroid, the duration of iritis averaged 3.8 mo. They showed favorable prognosis of vision. Of 29 patients (58 eyes) with delayed treatment, the visual acuity in 13 eyes deteriorated to less than 20/50. These 13 eyes showed prolonged inflammation in the anterior segment for an average of 45 mo. Cataract operation was performed in 12 eyes (10.3%) and peripheral iridectomy was performed in 9 eyes (7.8%). The cases with posterior uveitis without papillary edema showed good prognosis of vision. In the other cases, early treatment with systemic corticosteroid was necessary to obtain good prognosis of vision.
