Effect of reduced glutathione (GSH) on O2-Hb affinity of human red blood cell was studied in vitro and in vivo. In vitro addition of GSH 50 mg and 100 mg to 10 ml of blood did not change P50 [O2 partial pressure for 50% Hb saturation] which was measured by Hem-O-Scan (Aminco) and corrected to the standard condition by calculation. GSH (10 g) was drip-infused over 30 min to 8 patients who underwent relatively major surgeries of > 4 h duration. P50 significantly rose from 28.4 to 29.4 torr 1 h after the end of the infusion of GSH. The control group of 8 patients who underwent surgeries of similar type and duration did not show any significant changes in P50 when the same time course was followed for measurement. Possible mechanisms for this in vivo increase of P50 are discussed but none are satisfactorily proved at present.
