Downbeat nystagmus was generally associated with surgical lesions at the level of the foramen magnum or with medical causes, e.g., diabetes, multiple sclerosis and meningoencephalitis. Cases (4) of downbeat nystagmus in alcoholics, an association not well recognized and may form a distinct clinical entity, are described.
