Clinical picture of protracted depressive and depressive-paranoid states

A structural-dynamic analysis of 85 patients with protracted endogenous depressions was performed. In accordance with th clinical picture all the patients were divided into two groups: 1. with a polymorphic picture of a depressive state and deep psychopathological disturbances (45 cases); 2. with a relatively monomorphic structure of the phase (attack), a mild level of positive disturbances (pseudoneurotic disturbances 40 cases). The clinical picture was characterized by an atypicity, instability of the leading signs. Among the patients there was a prevalence of people with intravert traits, with a high hereditary loading. In the premanifest period, these patients showed alleviated disturbances of mental activity, leading to personality shifts. Following 3--3.5 years after the depression there was a tendency towards its chronification with noncompliance to all forms of therapy.
