Metriphonate and organophosphate poisoning

An 8-year-old boy was admitted to hospital in Rhodesia with a history of vomiting and sweating profusely about half an hour after taking metriphonate at a local clinic. He had been given a single dose of 250 mg (10 mg/kg body-weight). He was perfectly well before this apart from haematuria due to Schistosoma haematobium (viable ova in urine). The child recovered well on atropine (1.8 mg given intravenously over 15 min and repeated after 30 min). All the symptoms suggested organophosphate poisoning.
