Anatrophic nephrotomy for treatment renal staghorn lithiasis

Fifteen patients bearing renal staghorn calculi in 17 renal units were subjected to anatrophic nephrolithotomy and the usefulness of this procedure was evaluated with respect to immediate postoperative complications and long-term follow-up. Except for a significant decrease in the renal function in 1 patient having unilateral procedure, no other problems were seen in these patients. The postoperative course was uncomplicated in most of the cases, with only minor problems in 3 patients. The renal function was stable or improved postoperatively in 93% of the cases, the incidence of urinary tract infection decreased from 80-27% after surgery and stone recurrence occurred in no patient. Apparently, the anatrophic nephrolithotomy is a reliable method to approach staghorn renal calculi when more simple procedures like limited pyelotomy or extended intrasinusal pyelotomy cannnot be used safely.
