Hb synthesis of erythroid precursor cells in vitro was studied in 14 patients with anemia of chronic infection (ACI), 5 normal healthy controls (NC) and 10 patients with Fe deficiency anemia (IDA). The results expressed as Hb synthesis (.mu.g)/106 erythroblasts during an 8 h period of short term cultures showed significantly decreased values in patients with ACI. The defect did not involve Fe uptake by the erythyroid precursor cells which was normal, but seemed to be located at some subsequent intracellular stage of Hb synthesis. This defect did not appear to be due to preceding in vivo exposure of the red cell precursors to the hypoferremic environment of ACI.
