Demonstration of immune complex deposits by electron microscopy and immunofluorescence in a fatal case of hypersensitivity angiitis

A 63 yr old woman is reported with a fatal case of hypersensitivity angiitis demonstrated by deposition of immune complexes in renal glomeruli detected by electron microscopy at autopsy. Direct immunofluorescence techniques reveal widespread anti-immunoglobulin M and anti-complement antisera with irregularly staining mesangial glomerular capillaries. Disseminated arterial necrosis is characterized by fibrin deposits in the media and intima with marked neutrophilic infiltration. Pathogenesis of hypersensitivity angiitis is compared to other autoimmune mechanisms including poststreptococcal glomerulonephritis, Wegener's granulomatosus, membranoproliferative glomerulonephritis and hepatitis with a positive .beta. surface antigen.
