Drug test for differentiating ventricular and supraventricular paroxysmal tachycardia

An isoptin test was conducted in conjunction with esophageal or intra-auricular ECG as part of differential diagnosis for ectopic tachycardia. Retrograde conduction of impulses from the ventricles to the auricles significantly decreased or stopped after administration of anti-arrhythmic drugs (obsidan, isoptin, novocainamide). A case study of a man with cardiac ischemia, postinfarction cardiosclerosis, paroxysmal tachycardia and chronic circulatory insufficiency is presented. Auricular paroxysmal tachycardia was excluded from the diagnosis because the frequency of the QRS complex surpassed that of the P waves after isoptin administration. ECG of the His bundle is considered the most reliable of diagnostic methods.
