Etiopathogenetic considerations on 27 cases of ventricular tachycardia with torsade de pointe (TDP)

Of 4144 patients, 27 had ventricular tachycardia with torsade de pointe [TDP]. The statistically significant prevalence of arrhythmia in women was related to the longer Q-T duration and the more frequent U wave presence. There were multiple arrhythmogenic factors in the 27 cases. The arrhythmia, despite its polymorphism, could have a monocentric origin. The hypothesis that TDP originates with focal reentry or negative afterpotentials and enters the His bundle branches and the 2 divisions of the left branch was supported.
