Although IH has been recognized for more than 20 yr and accounts for 30-40% of patients with calcium stones, its pathogenesis is complex and its etiology obscure. Investigation has attempted to resolve these issues, and it is likely in the future that the term idiopathic will be eliminated and the disease understood in terms of 1 or more fundamental metabolic defects. Data from several workers suggest that there are several causes of hypercalciuria in stone formers. At least 3 types of hypercalciuria, renal, absorptive and resorptive, have been proposed.
